Medical Form for Summer Camp
Scouts going to camp for more than a week must complete the expanded medical form, including the Meningococcal Vaccination Response Form. This includes those Scouts going to Eagle Camp and regular summer camp.
Scouts that are only going to the one week of regular camp complete the basic part of the form.
